2008 Intel ISEF Forms
The following three forms are required for ALL participants: the Form 1- Safety Assessment Form, the Form 1A- Research Plan and Research Plan Attachment, and the Form 1B- Approval Form . All projects also require the Official Abstract (see below) and may require the additional forms listed below.
Abstract Form -- The Official Abstract Form is required for all participants. Finalists at the Intel ISEF are required to use this official copy of their abstract for display and handouts. Please read the instructions for completion thoroughly.
Specialized Forms that MAY BE REQUIRED.
Form 1C- Industrial Setting Form This form explains what the student researcher actually did and is signed after the project is completed. This form is only required if the project was done at a research institution (a university lab or industrial setting).
Form 2- Qualified Scientist Form Required for research involving animals, controlled substances and pathogens; may be required for rDNA, tissues, and humans. On this page, the scientist explains what will be done to oversee this project. The date signed indicates the date that they approve this project (before experimentation takes place).
Form 3- Designated Supervisor Form Required if the Qualified Scientist is unable to supervise the experiment OR for projects using hazardous materials or devices. The designated supervisor explains how the project will be supervised and what safety precautions will be employed. The date signed is the date that they approve this project.
Form 4- Human Subjects and Informed Consent Form Required for all research involving humans. This page is filled out by the student researcher to explain to the IRB how the safety and well being of the test subjects will be ensured. The IRB reviews the project, checks the risk level and each member signs with the date they approve this project. This review and the date signed must be BEFORE any experimentation takes place.
Informed Consent *Required for all subjects under 18 years of age (see exceptions); required for all projects involving risk; and recommended for all projects involving human subjects.
*Use a separate form for each test subject.
This form is filled out by the student researcher and explains very completely to the research subject and their parent (guardian) exactly what will happen to the subject in the project. Questionnaires, sample tests, and so on MUST be given to the IRB and to the parent/guardian. If they approve, they sign with the date that they approve. (Before the experiment begins). If a photo is to be displayed, the participant signs and dates it when they give permission.
Form 5- Vertebrate Animal Form (5A and 5B) Required for all research involving nonhuman vertebrate animals. This form is filled out by the student researcher and describes the housing and care for the animals. The bottom of the form is filled out by the supervisor or scientist and is signed and dated when they approve this project with these housing conditions. (Before experimentation begins.)
Form 6A- Potentially Hazardous Biological Agents, Form 6B- Human and Vertebrate Animal Tissue Form, Required for all projects using viable fresh tissue, organs, human or animal parts, including blood, blood products, teeth, primary cell cultures, and body fluids.
This form is filled out by the student researcher and explains the source of the tissue. The tissue provider signs in the appropriate boxes to certify the safety, acquisition, etc. The SRC must sign to show they approve the use of this tissue and the date (before experimentation) that they approve. If an established cell line is used or other exempt tissue (as described in the rules), the researcher may explain that on this page, but no signatures are needed in this case.
Form 7- Continuation Projects Form Any project conducted by the student or team in a similar area of research as previous projects should be considered a continuation. Explain as completely as possible how the project is different from previous experimentation because ONLY a new and different research project is allowed. (It can be based on previous research, but must be new and different research.) Date signed is the date the student researcher is certifying that this information is correct.
